Child Health Assessment: To be filled out by your child's pediatrician. You have until six weeks after your child's first day to return this form. For Preschoolers, the form must be updated annually.
Demographics Form: Tell us about your child!
Emergency Form: Your child must have an up to date emergency form on file to attend. Please remember to let us know if you move, change jobs, or change phone numbers so this form can be updated.
Lunch Enrollment & Meal Benefit Income Eligibility: We provide morning breakfast, lunch from SRU's dining hall service, and an afternoon snack. This form is required for every family because of our participation in State funding programs.
